While we look forward to receiving your request, such requests do not guarantee the nominated provider will become a participating CHP provider. In the event your provider nomination is denied or the provider opts not to participate, we will contact you; otherwise, we will initiate contact with this provider upon a nomination approval. As part of the credentialing and contracting process, providers are thoroughly vetted and must meet certain quality standards. This process can take up to 30-60 days from the receipt of the provider’s complete application, at which time the provider is notified and may begin treating members.
